¿QUE ES LA LIPOSUCCION?


Es una tecnica que permite modelar el contorno corporal mediante reduccion o aumento del tejido adiposo deacuerdo a la armonia corporal del paciente lipoescultura.


¿QUE TECNICAS DE LIPOSUCCION EXISTEN?


Existen diversos metodos de liposuccion:

LIPOSUCCION CONVENCIONAL:


Se realiza mendiante la aspiracion del tejido graso introduciendo una pequeña canula, conectada a una aspiradora de presion al vacio, dirigiendo la canula a las areas de grasa a ser succionadas por incisiones pequeñas (4mm).


LIPOSUCCION ULTRASONIDO


Las ondas de ultrasonido son transmitidas a la punta de la canula para licuar la grasa antes de ser succionada.


Aunque se promueve su utilización aduciendo menor traumatismo de ciertas estructuras, el calor que genera produce cicatrices mayores se aumento el riesgo de daño en la piel.


-VASER


Vibración amplificada de la energia sonora en resonancia intermitentes o continuos impulsos de energia ultrasonica se pueden utilizar para romper las celulas grasas que entonces son retiradas por la succion, dejando intactos otros importantes tejidos (como nervios, vasos sanguineos y tejido conectivo) y celulas madre ubicadas en los planos profundos del tejido graso teniendo el riesgo de aumentar de peso en 6 meses.

La liposuccion puede llevarse a cabo exitosamente solo con anestesia local, mediante la utilización de la formula de Klein por tal motivo usted va estar durante toda la operación conciente, se le acondicionara un ambiente agradable durante el tiempo que usted permanezca en sala de operaciones en el cual hasta podra conversar con el personal que se encuentre en sala de operaciones, a menos que usted lo solicite el anestesiologo de la clinica le administrara un ligero sedante.


¿QUE REQUISITOS DEBO CUMPLIR PARA SER UN BUEN CANDIDATO A LA LIPOSUCCION CORPORAL?


Debo cumplir con lo siguiente:


Consulta inicial. Entrevista medico – paciente.
En la entrevista el paciente debe quedar sastifecho con las repuestas a sus interrogantes.
Conocer el protocolo de seguridad de la clinica.
Comprender las indicaciones pre-operatoria, procedimiento de la lipoplastia y , las indicaciones del post-operatorio.
Tener un aceptable historial clinico, analisis pre-operatorio, riesgo quirurgico, evaluacion cardiovascular y ecografia en tejido celular sub cutaneo.
Las expectativas del paciente deben ser realistas.




¿COMO ME DEBO PREPARAR PARA LA LIPOSUCCION?




.debe encontrarse en ayunas 6 horas antes de la intervención.
.acudir con vestimenta y calzados comodos.
.debera comunicar al medico si ud ingiere fármacos (aspirina,antiinflamatorios,etc)
.si esta con su periodo menstrual debe consultar con su cirujano










¿CUANTO TIEMPO SE DEMORA UNA SESION DE LIPOSUCCION?




Esto depende de numero de areas que seran tratadas y de la cantidad de grasa que sera removida.
Por lo tanto, esto podria tomar desde 30 minutos hasta 2 horas.


¿SENTIRE DOLOR DURANTE LA LIPOSUCCION?


La liposuccionse realiza con anestesia local en la zona diseñada, se utilizara conjuntamente una sedacion leve con la cual el paciente se encontrara relajado y con sensación de sueño acompañado de estimulos dolorosos leves similares a un pellizco en la zona operatoria.


¿NECESITO PERMANECER HOSPITALIZADO?


No, porque la lipoplastia en la clinica arroyo se realiza por zonas nunca se hacen todas las areas a operar el mismo dia y se emplea anestesia local.


¿CUANTO DE GRASA SE EXTRAERA?


Es importante conciderar que existe un limite maximo seguro en la cantidad de grasa que puede ser removida durante la operación la cual es de 1 litro por sesion operatoria.es importante comprender que el cirujano no arriesgara su seguridad al remover mas de lo que sea adecuadamente seguro.


¿REGRESARA LA GRASA LA CUAL FUE EXTRAIDA EN LA LIPOSUCCION?


La respuesta directa a esta pregunta es “no”.como las celulas adiposas han llegado al limite de acumulación en cualquier parte del cuerpo durante la pubertad, el aumento de ingestión de grasas no conlleva a un aumento en el volumen del area tratada. Es importante, sin embargo, mantener su peso mediante una alimentación moderada y ejercicio regular. Debe entender que si usted come calorias en exceso después de la cirugia, notara que otras areas de su cuerpo aumenta de tamaño, ES DECIR UD NO VUELVE A ENGORDAR LAS ZONAS TRATADAS.


¿CUANTO PESO PERDERE?


Durante las primeras semanas después de la cirugia puede notar un ligero aumento en su peso debido a la retencion de liquidos. Esto se debe a que las celulas adiposas no pesan tanto. Por otro lado, sin embargo, dependiendo por supuesto del area a ser tratada, usted notara una disminución significativa en sus medidas.


¿QUE SUCEDE INMEDIATAMENTE DESPUES DE LA LIPOSUCCION?


Después de su cirugia, usted sera dirigida(o) a la sala de recuperacion en donde permanecera por el lapso de 20-30 minutos y se le dara las siguientes indicaciones:


podra ingerir alimento 2 horas después de la cirugia
debera rehidratarse en las proximas horas post operadas (bebidas rehidratantes 2 a 3 litros)
no podra consumir alcohol, aji, pescado, mariscos, cerdo, vitamina E durante 15 dias
no debe fumar durante 15 dias


Transcurrido el tiempo de reposo ud . estara en condiciones para retirarse a su domicilio.


¿QUE TIPOS DE EFECTO PUEDE ESPERAR DESPUES DE LA LIPOSUCCION?


Después de la liposuccion la cual varia de 30min a 2 horas dependiendo el area de la cual fue intervenida usted notara que drenan un liquido a traves de sus diminutas incisiones (el cual es de un color rojo claro). Esto se debe a que existe cierta acumulación de liquidos en el area tratada. En los puntos de incisión se dejara los drenes por lo tanto, actuaran como “orificios de drenaje”, siendo muy importante para reducir inflamación y las contusiones postoperatorias.


¿COMO LUCIRE INICIALMENTE?


Es importante entender que el tiempo de recuperacion varia dependiendo del paciente.
El dia siguiente a la cirugia se recomienda caminar en la casa para promover la circulación sanguinea.
Notara inflamación el cual suele mejorar a la semana y las equimosis (hematomas) pueden durar hasta tres semanas o mas.


Algunas areas pueden sentirse deformes y toma algunas semanas antes de retornar a su normalidad.


¿COMO ME ENCONTRARE EN LOS PROXIMOS MESES DE LA LIPOSUCCION?


El cuerpo comienza a formar tejido de cicatrizacion en repuesta a la lipoplastia, la cicatrizacion cesa y se elimina por completo, siendo perceptible a la vista. Sin embargo, este periodo de endurecimiento e irregularidad puede tardar en desaparecer de 3 a 6 meses.


¿TENDRE CONTROLES POST-OPERATORIOS CON EL MEDICO?


Si, usted tendra controles y curaciones post-operatorias al dia siguiente y las veces q su medico lo indique para poder evaluar su progreso. Si tiene preguntas y preocupación durante su recuperacion o necesita imformacion adicional, usted debe regresar a ser atendido por el staff medico de nuestra clinica.


¿QUE APARIENCIA TIENE LA CICATRIZ DE UNA LIPOSUCCION?


Afortunadamente, las cicatrices en la lipoplastia son minimas. Se realizaran incisiones las cuales se colocaran en los pliegues naturales del cuerpo escondidas cuando sea posible y dependiendo el area a ser operada se requiere por lo general de 2-3 incisiones para cualquier area. Debido a ello se formaran cicatrices de aproximadamente 3-4mm las cuales sera imperceptible en su totalidad al año.


¿QUE COMPLICACIONES PUEDEN PRESENTARSE EN UNA LIPOSUCCION?
Afortunadamente las complicaciones en clinica arroyo no se presentan debido a que se maneja el protocolo arroyo de seguridad en lipoplastia. Cada año se someten miles de pacientes a la liposucción sin complicaciones mayores y con alto grado se satisfacción personal. What is PRP

PRP is an acronym for Platelet Rich Plasma. Plasma is essentially blood without the red blood cells. Platelets are cells found in the blood that are essential for clot formation and wound healing. PRP, the mix of concentrated platelets and serum, contains factors that promote repair of tissues. PRP is injected into the targeted area for repair.

Limit Mistakes in Liposuction Clinica Arroyo Goldstein Lima Peru


How do we limit the risk of complications in Liposuction.

Our procedures and protocols have been in place, and used without question for almost 30 years. Some people might think that our measures are extreme, however that is why our clinic can boast about our success rate. It is not inconvenient to do Liposuction in several sessions over alternating days, it should always be the rule. Many Doctors and clinics are just too eager to take a patients money, and only tell them what they want to hear, or let the patients make their own decisions based on their personal schedules. That will only lead to problems, and sometimes dire consequences.

Safety of the patient is our first priority, that is why we do not accept all patients for surgery. If a patient is at risk, first an assessment will be done with a Doctor who specializes in that particular area( such as a Cardiologist for a heart related condition, etc) If all is approved, surgery will be done in accordance to any specialized instructions.

The greatest risks of liposuction are associated with the tendency of some surgeons to do too much surgery on the same day. The three types of excessive surgery include 1) removal of an excessive volume of fat by liposuction on a single day, 2) liposuction of an excessive number of body areas on the same day, 3) combination of liposuction and other unrelated surgical procedures which involves excessive surgical trauma and prolonged exposure to general anesthesia.

For those reasons as stated above, Only local anesthesia will be used, no more than 4 flasks of lidocaine or other anesthetic will be used at one time, no combination of surgeries will be done on the same day, Liposuction will be limited to particular zones.

A moment about local anesthesia and the safety of Tumescent Liposuction.

Lidocaine is the safest of local anesthetics available for liposuction. At lidocaine concentrations in the blood exceeding 6 milligrams/liter, patients might experience some toxicity. Blood concentrations of lidocaine in excess of 12 milligrams/liter can produce serious cardiac toxicity. The maximum recommended dosage of lidocaine for tumescent liposuction is 50 mg/kg (50 milligrams/kilogram of patient weight). The most extraordinary aspect of the tumescent technique is its unprecedented safety record when used as directed. Most liposuction surgeons know that it can be dangerous to give a patient a dosage of tumescent lidocaine that exceeds 50 mg/kg. All liposuction-related deaths have been associated with either the use of general anesthesia, or IV sedation, or lidocaine doses in far in excess of 75 mg/kg. The tumescent technique is dangerous in the hands of surgeons or anesthesiologists who have not had specific training in the technique. There has never been a reported death associated with tumescent liposuction totally by local anesthesia.

Preventing complications should be the most important concern of both patients and liposuction surgeons. Surgeons can reduce the risk of liposuction complications by proper selection of patients. Surgeons should avoid patients who have 1) unrealistic medical expectations, 2) a history of significant medical problems. Patients can reduce the risk of complications by 1) resisting the temptation to save time by attempting to do multiple unrelated surgical procedures along with liposuction, and 2) resisting the temptation to maximize the volume of fat removed on a single day. It is much safer to divide one very large liposuction procedure into two or more separate procedures done on separate days. Liposuction by general anesthesia can be considered safe, however, liposuction totally by local anesthesia is safe.

Introduction To Liposuction
"Liposuction is the surgeon's method of sculpting the body contours."

Liposuction is the most common cosmetic surgical procedure performed in the United States today and works best when treating localized fat deposits. Adipose tissue is surgically removed from specific areas through small skin incisions using a hollow metal tube attached to a vacuum apparatus. You are an ideal candidate for liposuction if you are at or close to your ideal body weight, and you want to improve the contour of your body but you have not been previously successful with traditional diet or exercise. Liposuction can slim the hips, and thighs, can flatten the abdomen and can remove excess fat from the chin and upper arms. The treated areas will retain their new contour as long as you do not have large weight gains.



There are certain areas of the body, such as the upper arms, abdomen and inner thighs, that may develop loose or hanging skin following liposuction. The degree of skin retraction and tightness depends on the patient’s age, the amount of fat to be removed and the patient’s general skin tone and degree of elasticity. If you are at high risk for loose skin, we may recommend a skin tightening procedure such as a thigh lift, tummy tuck or upper arm lift. If it is equivocal whether the patient will develop skin laxity after liposuction alone, removal of excess skin can be performed at a later date.



Fat cells play numerous metabolic roles in the body. They serve as an energy source, a storage place, and interact with insulin and hormone synthesis, to name a few. Adult fat cells are thought to be incapable of multiplying. There are a fixed number distributed in a genetically predetermined fashion throughout the body. Regardless of the function, as you gain weight these cells expand. As you lose weight, they contract but the number and distribution remain essentially unchanged. This is why thin people may still complain about localized fatty deposits that don't go away even at their ideal weight. Dieting reduces your weight and overall size, and may show improvement in specific areas. Liposuction reduces the overall number of fat cells, and affects shape and contour.



Liposuction has come a long way since its introduction in the mid 1970’s and modern techniques have made it safer and simpler for both surgeons and patients. It is a safe and effective way to remove unsightly bulges from almost any area to produce an improved shape and contour. This is why liposuction has consistently been the most popular cosmetic surgical procedure in the world for the past several years.



Fat deposits that don't respond to the usual litany of exercise and diet regimens are ideal targets for liposuction. If you are only overweight in certain areas of your body, you would have to lose a larger amount of weight in order to shrink the size of your thighs. The weight will come off everywhere including the breasts and face, and not just where you need it most. Most body parts can be suctioned for better contour and reduced volume, from the face down to the ankles. The most popular areas for women are the abdomen, inner thighs, outer thighs, hips,and flanks. Liposuction can even be used to reduce heavy breasts in some women.



Before having liposuction, we will discuss your lifestyle, aerobic shape, and body weight fluctuations. If you have a history of an eating disorder like bulimia, anorexia, binge eating, or have been on prescription weight control medications, we need to know in advance. If you are overweight you may have to undergo these procedures in stages. If you exceed your ideal weight by 50% or more, we may suggest that you lose weight prior to undergoing a liposuction procedure.



Liposuction of multiple areas should not exceed five liters of total fat removed in the outpatient setting and 10 liters in the inpatient setting. When large volume liposuction is performed, it is best to treat "aesthetic units" of the body to maximize the result. For instance, it is possible to combine liposuction of the back, love handle area and abdomen during one session, and the hips, inner and outer thighs and knees during another session.


How We Do It
Advances in liposuction techniques have greatly improved the results you can achieve. The procedure is very straightforward. Tiny incisions of approximately one quarter inch long are made at the sites where fat is to be removed and a wetting solution is infused to provide anesthesia, reduce bleeding and improve fat extraction. This requires careful monitoring to avoid toxicity and must be performed by those experienced with local anesthesia. The surgeon uses various sizes and dimensions of cannulae, hollow, tubular instruments with holes at one end to trap the fat. The cannula is attached to suction tubing through which the excess fat is evacuated.



These instruments come in various shapes, lengths and sizes depending on the thickness and location of the fat. They have highly polished surfaces to slip through the fatty tissues with minimum friction or damage, and are frequently blunt-tipped to prevent cutting through the skin. Cannulae are inserted under the skin moved in a back and forth and criss-cross fashion within the fat, essentially pushing it aside while protecting the vessels and nerves. Fat is suctioned out through one or several holes at the tip, measured, and then the patient is checked for symmetry. The procedure is completed when a safe level of fat removal and the desired contour has been achieved. The patient is then monitored closely to make sure that enough fluid hydration has been received.


The 'wet' techniques, and the superwet technique, referring to the amount of fluids injected, are variations of the tumescent technique. Tumescent anesthesia has had perhaps the most significant impact of all developments in liposuction. Warmed tumescent liquid, a diluted solution containing lidocaine, epinephrine and intravenous fluid, is injected into the area to be treated. As the liquid enters the fat, it becomes swollen, firm and blanched. The expanded fat compartments allow the liposuction cannula to travel smoothly beneath the skin as the fat is removed. The saline softens the fat, the adrenaline decreases the blood loss and bruising, and the anesthesia provides relief from discomfort.

Ultrasound Assisted Liposuction (UAL)
Ultrasonic sound waves, like shock waves, are transmitted into the fatty tissues from the tip of the cannula probe. The fat cells are melted or liquefied and then removed by low-pressure vacuum through a suction tube. Ultrasonic liposuction is often reserved for more difficult areas to contour where the deep fat is thicker, and thus harder to extract; i.e. back rolls, upper abdomen, and flanks.
